Indication for Labetalol

Population group: only adults (18 years old or older)
Therapeutic intent: Curative procedure

Mild, moderate or severe hypertension.

For this indication, competent medicine agencies globally authorize below treatments:

100 mg twice daily followed by 100-400 mg twice daily or 2400 mg daily divided into 3 or 4 doses

For:

Dosage regimens

Oral, 100 milligrams labetalol, 2 times daily. Afterwards, oral, between 100 milligrams labetalol and 400 milligrams labetalol, 2 times daily. The maximum allowed total dose is 2,400 milligrams labetalol daily.

Detailed description

Treatment should start with 100 mg twice daily. In patients already being treated with antihypertensives and in those of low body weight this may be sufficient to control blood pressure. In others, increases in dose of 100 mg twice daily should be made at fortnightly intervals. Many patients' blood pressure is controlled by 200 mg twice daily and up to 800 mg daily may be given as a twice daily regimen. In severe, refractory hypertension, daily doses up to 2400 mg have been given. Such doses should be divided into a three or four times a day regimen.

Elderly

In elderly patients, an initial dose of 50 mg twice daily is recommended. This has provided satisfactory control in some cases.

General

If rapid reduction of blood pressure is necessary, see the SPC for labetalol injection. If long-term control of hypertension following the use of labetalol injection is required, oral therapy with labetalol should start with 100 mg twice daily. Additive hypotensive effects may be expected if labetalol is administered together with other antihypertensives e.g. diuretics, methyldopa etc. When transferring patients from such agents, labetalol should be introduced with a dosage of 100 mg twice daily and the previous therapy gradually decreased. Abrupt withdrawal of clonidine or beta-blocking agents is undesirable.

Labetalol lowers the blood pressure by blocking peripheral arteriolar alphaadrenoceptors thus reducing peripheral resistance, and by concurrent betablockade, protects the heart from reflex sympathetic drive that would otherwise occur.
