Interactions

Econazole interacts in the following cases:

Vitamin K antagonists

Econazole is a known inhibitor of CYP3A4/2C9. Due to the limited systemic availability after vaginal application, clinically relevant interactions are unlikely to occur but have been reported with oral anticoagulants. In patients taking oral anticoagulants, such as warfarin or acenocoumarol, caution should be exercised and the anticoagulant effect should be monitored more frequently.

Adjustment of the oral anticoagulant dosage may be necessary during and after the treatment with econazole.

Pregnancy

In animals, econazole nitrate has shown no teratogenic effects but is foetotoxic at high doses. The significance of this to man is unknown as there is no evidence of an increased risk when taken in human pregnancy. However, animal studies have shown reproductive toxicity.

Systemic absorption of econazole is low (< 10%) after topical application to the intact skin in humans. There are no adequate and well-controlled studies on adverse effects from the use of econazole topical cream in pregnant women, and no other relevant epidemiological data are available.

Because there is systemic absorption, use of econazole is not recommended during pregnancy.

Nursing mothers

Following oral administration of econazole nitrate to lactating rats, econazole and/or metabolites were excreted in milk and were found in nursing pups. It is not known whether econazole nitrate is excreted in human milk. Caution should be exercised when using econazole 150mg vaginal pessaries if the patient is breast-feeding.

It is not known whether cutaneous administration of econazole topical cream results in sufficient systemic absorption of econazole nitrate to produce detectable quantities in breast milk in humans.

A risk to the breast-fed child cannot be excluded.

A decision must be made whether to discontinue breast-feeding or to discontinue/abstain from econazole topical cream therapy taking into account the benefit of breast-feeding for the child and benefit of therapy for the woman.

If econazole topical cream is used while breast-feeding, care should be taken to ensure the cream is not applied to the nipple or surrounding area.

Carcinogenesis, mutagenesis and fertility

Fertility

Results of econazole animal reproduction studies showed no effects on fertility.
